Do we have the option of making the patient portal pages less like a website from the 1990's? I know the login is hosted through Crystal's servers but was wondering if I have the option to update the look like I could with out online patient forms.

Can you please tell me how to add a link to my office web page so patient's can complete their paperwork online and then it can be uploaded into their chart without them printing the form and bringing it in and then we have to type all the things they have written? Thank you

Thank you You will need the URL for you patient portal which you can find by going to Admin->website There are 2 URL's listed. The first one www.crystalpm.com/index.jsp?crystalpmid=#### is for the online forms. The second one www.crystalpm.com/PatientPortal.jsp?crystalpmid=#### is for the patient portal. You will need to get in contact with your website designer with the URL(s) (or whoever manages your website) and let them know you would like buttons made.

Thank you Are you just wanting to know how to link someone from your website to the online forms? If so the link to your online forms can be found under Admin-->Website... Your webmaster, or whoever administrates your website, should be able to create a hyperlink on your website that will refer anyone who clicks it onto the online forms pages... However, You will need to configure, or have some at Crystal configure, your online forms first so that data entered online will transfer to the corresponding fields in your record template...

Just double checking, as a staff member had this question. A patient is assigned a Username & Password to access Patient Portal within Crystal PM by our office and then once in the portal, they can not change the password?

They aren't able to change it on the portal themselves currently. They would need to contact you to update it.
