We have not been able to get a Post-Op Cataract or Yag paid by Medicare since starting Crystal and Apex. We have been shown several different ways, however none have been successful. We am hoping someone who has been successful would advise me. Medicare EOB always state Claim/service lacks information or has submission/billing errors which is need for adjudication. We have spoken with Medicare during claim processing and they state everything looks good and there should be no problems but.....they are denied. We are now over a year for some.

Things to check - place of service = home; referring dr is listed with DN in box 17; line 19 has glasses post cataract surgery and dates of surgery (11-7-16 - RT, 11-8-16 - LT);diag code = z96.1 (Bluecross prefers a cataract code). I sent you a private message with my number if you want to call me and go thru your claims together.

We have been pretty successful at getting payment for Post Cat and Post Yag appointments, and we also use Apex as a clearing house. If you are still needing help you can contact us at frontdesk@puyallupvision.com

What is happening with your claims?
