New template for printing CMS claim without background

We have two practices. And they have different printers and when I try to print a paper claim on a CMS 1500 I have to change the default setting all over again using an admin account. I asked Ryan if there was a way to add a second default but he said there isn't and to request it here, so here I am.

Unless you guys can let us customize the format of every field on the CMS form. For some reason different printers print with different defaults and being able to adjust only the Left margin and Top margins while trying to make all the fields correctly print within their boxes is frustrating.

I have the same problem.  I can't print claim forms for my remote locations.  No matter how I try to change the default settings, it does not line up. 

I have been filling a form at my location under our fake test patient.

What if we had an option to fill a blank form?
