I currently use a very old and outdated medical record template. Before I dive head-first into the hundreds of templates in the database I wanted to poll the audience...what does everyone else use? I know about Marcia's template, and it looks fantastic, but I wanted to see what other popularly used templates are out there.
Secondly, in the template database is there any way to know when a specific template was entered/updated? I'd really prefer to not waste time by uploading a template that is 10+ years old.

Most of our offices are using Marcia's template or a version of Dr. Cass' template (Beaumont Family) We took Dr. Cass' original template and added a few things to it - this is known as the Crystal Template. The Crystal Template is getting a little face lift ETA is hopefully a few weeks (maybe sooner!) I attached a PDF of screenshots so you can see a visual. Crystal_SS.pdf (654.09 KB) *update, the final template has been released - please email if you would like more info* Unless the office specifies the date they are uploading their template there is not a way of knowing how recent it is. If you want to test drive any of the templates before implementing them into your practice, we can load the Crystal demo on one of your workstations or a laptop. A laptop is nice because then you can take it home. A few things to keep in mind about switching your template: If you have online forms set up (patient’s fill out their medical hx, insurance and demographic info online) they would need to be reconfigured and this is about 3-4 weeks out. Any machines that you currently have integrated would need to be re-integrated as well but this is usually something that you can schedule 1-2 days out. Re-configuring the online forms and the machines may incur a charge so it is best to talk to tech support before switching your template. The other major point to bring up is your previous records. We will turn on what is called versioning. This will preserve the previous records, allowing you to view your old records in the template they were created in. You will not be able to use the auto history or previous buttons when you switch to your new template until you have seen the patient once. You will still be able to import previous final glasses and CL prescriptions. I hope this helps! Our office uses a modified version of Dr. Cass's Template.. In my opinion you guys are both spot on, for new users I would suggest starting with Dr. Detloff's template or Dr. Cass's and making necessary customizations where you see fit.
Dr. Detloff has a very thorough and comprehensive template. It is already heavy on customization, and many of the automated processes she has built in are not present in Dr. Cass's basic template. The learning curve will probably be a bit steeper with your staff if you decide to use her template, however that may be easier than the alternative of using Dr. Cass's template and making a bunch of customizations if no one on your staff is that computer savvy.
If you or a staff member is rather computer savvy, and you would like a more tailored experience I think Dr. Cass's template is a great foundation for you to start with and build customizations off of. That is what I chose to do with our template and it has worked really well for us. That said, I did take a significant amount of time customizing it and adding features like buttons that automate processes that Dr. Detloff's template already had built in.
Both templates should have sufficient capability to stay meaningful use compliant if that is a concern for you as well.
You can also check out our template if you are interested. Like I said it is based on Dr. Cass' template but customized and updated for ICD10 should be listed under "Frisco Eye Associates"

I agree with you Frisco!! Marcia's template is AMAZING!! It has all of the bells and whistles and I would guess that all together she has spent 200+ hours on it! Out of the box, it's a solid template and it just takes a little time to get acquainted with all of the features. That being said, it can be a little overwhelming to some offices and we do not support modifying it other than changing F9 drop downs and maybe adding a few additional fields. If you modify her template, it can break the integrated web of features. If offices want a template to put their own spin on, we start with Dr. Cass' template or the Crystal template as they are ICD10/MU ready! It's a great base and is easy to modify

Out of the box, it's a solid template and it just takes a little time to get acquainted with all of the features. That being said, it can be a little overwhelming to some offices and we do not support modifying it other than changing F9 drop downs and maybe adding a few additional fields. If you modify her template, it can break the integrated web of features. If offices want a template to put their own spin on, we start with Dr. Cass' template or the Crystal template as they are ICD10/MU ready! It's a great base and is easy to modify My template is indeed pretty complicated and people that don't understand programming and aren't willing to put in the time to read the documentation I have shouldn't be making changes. That said, other templates don't have a lot of the features so, if you can't do it yourself, you aren't losing anything if you don't use the specialized buttons and it still has a good layout that works well for me. IMO, the advantage of my template is that there's not a ton of duplicate and unused fields because I started completely from scratch. I also included "future" fields so that I could keep my fields roughly organized (although I've run out of future fields in some areas). One of my priorities was to avoid scrolling so I resized all the text boxes to a smaller font size (because the default font size in Crystal is HUGE) allowing me to fit a lot more fields and I made all the Rx fields the min size boxes required to fit the max Rx. All the FIDs have descriptions so you know what the is and/or does. And I have a bunch of supporting information about what each button does and how to make typical customizations for those that are willing to put in the effort. I also have several Word templates and an online history form and I keep a log of changes I've made so people that are using an older version can add new features on their own if they want., without having to update the template and redo their customizations. Marcia Hi Marcia, I am looking at using your template and it is quite an amazing bit of work. But I don't see any letters in your template file. Am I doing something wrong where I just don't see them? Or, do you have letters created that could be uploaded to Crystal so I could import so the links are all correct with the FIDs you used? Karl, I run a Crystal User Group and I keep the latest template and all supporting files/documents on the Google Site associated with that group. If you want to join the Group, send me a message through the forum with your name, email address (gmail is best), office address and number and Crystal ID and I will add you to the User Group so you can access the files. FYI, I just uploaded an update with CL parameter buttons and changes in the CL replacement button to accommodate the Oasys 1 Day lens.
