Thought I would share this. New doc in office was writing a note on a new glaucoma diagnosis and wanted to convey IOP of "less than or equal to" his target number. His note kept disappearing upon closing the record. We narrowed it down to the use of "</=" in his note. That combination must tell the SQL database to clear the field. He ended up typing the phrase out in words rather than symbolically.

Lol, I've only ever seen one other doctor do that, but yes, that's what happened. Unfortunately the only solution is to change your data.
