Patient names and commas on new CMS form

On the new CMS form, the patient names are appearing as follows:

Doe,John, L

I know that Medicare and some other insurances are VERY particular about how the patient names are listed on the CMS form and will deny claims if they are not entirely correct. Will you please add a space after the last name and take out the comma after the first name so that it appears as follows:

Doe, John L

Besides, the way names are appearing currently don't make any sense. Why would there not be a space between the last name and first name and why would there be a comma after the first name? Please fix. Thanks.

The commas are there because the instructions on the form say that the name should be formatted (Last, First, Middle Initial). The space between the comma and the first name can be fixed. Look for it in an upcoming release.
