Hi! For PQRS Measure 226, I was wondering if there was a way to link smoker and no smoker codes to claim form once we enter their status on the medical records page. I am able to link codes when pertaining to a certain diagnosis but since smoker and no smoker are not a diagnosis, I can't link them the same. I am trying to save time and also do not want to miss putting this code on the claim form. Thanks!

May I ask why you are including smoking status on the invoice? Crystal tracks status for meaningful use through the ARRA tab. I have linked the smoking status on the patient's medical history tab to the ARRA tab so it automatically updates ARRA. Does this help any?

Hi Karen, We are adding smoking status for the 2014 Physician Quality Reporting System (PQRS). It is one of the 9 measures applicable to optometry. Thanks for the reply. We also have our smoking status linked from medical records to ARRA for meaningful use. It definitely saves time.

Maybe I'm just showing my ignorance here, but the only claim form I'm familiar with is the 1500 HCFA form. I don't think there is any reference to smoking status on this form. Can you tell me which form you're referencing?

Yes, I am referring to the 1500 HCFA Form. If I'm not mistaken, we need to apply codes 1036F and 4004F on the claim form for PQRS Measure 226. We are just starting PQRS this year and we plan on reporting on claim forms.

I know exactly what you mean about PQRS, and adding the smoking and meds recorded to the invoice, which is claims based reporting. We add both those codes by hand at checkout for Medicare patients, there doesn't appear to be a better way to do that we could figure out. The other codes are linked to a diagnosis so it pops up the question when working in the invoice. We created a small slip of paper with the the possible answers to each of the PQRS questions on each Medicare patient's chart, the technicians check the correct answers (including those for smoking and meds) and the receptionist makes sure that the correct codes are on the invoice in order to get "credit" from Medicare.

Peter, With PQRS, there is a procedure code for each of the PQRS items you are reporting that must be added to the claim form (so they need to be on the invoice). The PQRS items all used to be related to specific diagnosis codes and you could put links for the PQRS line items in the diagnosis codes. Apparently they have now added smoking and meds recorded to the list of PQRS items and since they are not associated with a diagnosis, it isn't coming up.
Since MU I haven't paid much attention to the PQRS stuff. These same items seem to be covered by the MU criteria and when you apply for MU, I thought you had to check that you were not applying for the PQRS incentive or you couldn't qualify for the MU incentive. Do I have that wrong or are you just doing the PQRS incentive instead of MU?
If I understand correctly, it sounds like it would be easiest to add some buttons to your med rec template that create a billing line item for the smoking and med PQRS items. Hit the button for the PQRS item you performed and it adds it to the invoice.
