On our Clinical Summaries the doctor's name appears with Dr. in front and O.D. in back. This is incorrect usage, how can we remove the Dr. from the front? Bad enough it shows his name twice, for the provider and the office. So we have Dr Lewis Kizer OD of Lewis Kizer OD. I see under Admin > Employees his name listed as Dr Kizer, Lewis, but when I open up his details, there doesn't seem to be anywhere to select/deselect Dr.

Once you check off "Is this employee a provider" in Admin->Employees, the system tags that employee as Dr. There is not a way to turn this off.
Where all of this pulls from: Dr. = In Admin->Employees, he has "Is this employee a provider" checked off Lewis Kizer = First, Middle & Last name in Admin->Employees OD = Suffix in Admin->Employees of Lewis Kizer OD = Company Name in Admin->Company

Doesn't this create a problem on reports, etc if I remove OD from the suffix field? You are not supposed to have both Dr and OD at the same time.

I'd leave it as Dr. Lewis Kizer and leave out the O.D. In the employee page click over to schedule tab and use unique name to Lewis Kizer, O.D.
Recently, I had a doctor who went through an audit and said they had to have it as Dr. Lewis Kizer, O.D. to pass (which I don't agree with) and can't confirm if that's accurate or not.
