We haven't filed a secondary insurance claim since Medicare started automatically forwarding claims to the secondary plan but I have a patient that has BCBS and Medicare is his secondary insurance. Another office said I have to send a paper claim to Medicare with a copy of the BCBS EOB but how do I get Crystal to generate the paper claim with the correct insurance info on the claim?

When you transfer the balance to Medicare, the CMS will populate the lines. You have to print on a real 1500 form - MC won't accept the Crystal copy. I'm not sure what you are asking about the 'correct insurance info', I hope this helps.

By the "correct info" I was referring to the having the primary insurance info listed in boxes 1,4, 7 and 11 and the secondary insurance info in Box 9 (Other Insured). I just tried a sample transaction with BCBS filed as primary. When I entered the BCBS payment and transferred the balance to Medicare, it did generate a CMS form addressed to Medicare but it placed the Medicare info in boxes 1,4, 7 and 11. And it has NONE in box 11. My OLD claim notes (as in 20 years ago) say box 4,7 and 11 only apply to insurance that is primary to Medicare and NONE must be written in box 11 UNLESS Medicare is NOT primary. Will that still work? Do I need to remove NONE from box 11. Marcia

I looked at one of my MSP claims. Boxes 1, 4 and 7 have the patients MC info, box 11 says 'none'. I believe you can submit secondary claims electronically now. We don't have very many so I still paper claim. If you try electronic, you have to add the primary ins payment info in Add'l Ansi instead of attaching the EOB.

Karen The additional info area will also add the bcbs as primary and medicare as secondary...I have a similar issue with VSP and medicare. If I looked at the area, I would be able to tell you exactly what to put where. I was able to figure it out by accident so it must be pretty easy to set up...just sayin' I am not the smartest bulb on 2.5 hours of sleep.

I was referring to whether Medicare will accept secondary claims, not the ability to do it in Crystal. Sorry I wasn't clear. Most of the time, the additional ansi section takes a little trial and error to make your numbers add up correctly. If you have CPM set up to import your EOB from your clearinghouse, it does a nice job of filling in the correct blanks. Not all insurance companies we deal with accept electronic secondary. I believe Medicare does now, but you have to add the reason MC is secondary in the Add'l Ansi section.
