is there a way to add "Self" option for "relationship to insured" when we click "not primary on account". right now theres only "spouse, child or other" if we accidentally check "not primary on account" and then click spouse but later its under patient/self there is no way to unclick spouse and it will continue to show spouse even if we uncheck "not primary on account"

I have had occasions where the cms form asked for relative information that I couldn't find. I thought this might explain it, so I played with the insurance tab. I was not able to replicate what you describe. Did you regenerate after making the change back to self? When I did, it came through correctly. Not sure what I did differently.

Yes, I can not re create issue either. Once you change patient back to self covered you should have no issues with it continuing to show spouse.. As Karen mentioned, you will need to "ReGenerate" the CMS form by clicking the button on the bottom left, but that should do it. If you still have issues I would consider looking at the Insurance's set up in Admin.. That is the only other thing I can think of... For instance, when setting up a new insurance company you can select something like the "medicare" radio button by mistake if it is not a medicare plan and this can affect the way information is pulled into the CMS form. In the case of checking that medicare button, it automatically fills each CMS form as a self covered form even if you have accidentally selected "Not Primary" on the patients insurance page, because you can only have self coverage with medicare.. Hope that makes sense...

i'm referring to the area the arrow is pointing to in the picture. its not a problem for us in submitting claims but we print out patient demographics and once this area is click its not possible to unclick. for example, one year the insurance is under the spouse but the following year its under the patient so we will uncheck "not primary on account" but because "spouse" was clicked on the previous year even though we uncheck "not primary on account" the "spouse is still check "behind the scene" so when we print out the demographics it will show "spouse" rather than "self"
Does anyone from Crystal know how to uncheck this "behind the scene" or can you please put in a option for "self" in the future update
