I checked my Meaningful Use Report today and saw a miss for Medication Reconciliation. Looking at that patient, I saw Pt Transferred In/Referred To This Provider was checked on the ARRA tab. Knowing he was not referred by a doctor, I looked at the referrals on the Additional Info tab of the Patient section and saw a patient referral from his daughter. Why would this call for a medication reconciliation? Shouldn't just the doctor referrals count? Are we using this section incorrectly? In addition, we have had several referrals from United Healthcare that we are trying to track using the referral section -> other referral. I added UHC as a default for the drop down selection to make it uniform. However, 1 patient out of 9 will not show up on our list no matter what we do. Any ideas why?
